Welcome to the Licensure Gateway!

Here’s how to renew:

• Log in with your username and password.
• Once logged in, scroll down to Licenses & Certificates.
• On your certificate, you will see Expiring Soon! Click to renew.
• Once you click the banner, it will take you to new page.
• Under License Options, click the rectangle entitled Renewal.
• Follow the prompts to renew.
• To print your 2025 certificate, click on HOME, at the top of the page, and scroll down to Licenses & Certificates.
• Upon completion of the renewal, a receipt will be automatically emailed to the address on the payment page.

To renew, you must log in under the permit through the FACILITY MANAGEMENT tab:

• Under the Facility Management tab, click the drop down and choose either Pharmacy or DME.
• Enter in the 5 digit permit number.
• Enter in the password (last 4 digits of the permit number).
• Once logged in, click on Renew Permit.
• Follow the prompts to renew.
• Once complete, scroll down to the EXIT button which will take you back to the management page.
• To print the 2025 permit certificate, scroll down to Options, and click on Print Annual Certificate.
• Upon completion of the renewal, a receipt will be automatically emailed to the address on the payment page.
