In order to access information about a licensee, you must first choose a license type. For licenses that apply to people you can search by license number, first name, and/or last name. For permits that apply to facilities you can search by permit number, business name, location city, and/or location state.
On January 28, 2021, the US DHHS issued a PREP Act declaration authorizing “any healthcare professional . . . who holds an active license or certification permitting the person to prescribe, dispense, and/or administer vaccines under the law of any State” to “prescribe, dispense or and/or administer COVID-19 vaccines in any State” as “long as the license or certification of the healthcare professional has not been suspended or restricted by any licensing authority, surrendered while under suspension, discipline or investigation by a licensing authority or surrendered following an arrest, and the individual is not on the List of Excluded Individuals/Entities maintained by the Office of Inspector General.”
A North Carolina pharmacy or other COVID-19 vaccination site that wishes to confirm that a pharmacist vaccinator from another state has no disqualifying disciplinary action is advised to consult: (a) the board of pharmacy in the pharmacist’s home state(s) of licensure (many have on-line verification tools); or (b) the below list of pharmacists holding a current NABP Passport (a pharmacist holding an NABP Passport has been vetted for disqualifying disciplinary actions).
The North Carolina Board of Pharmacy provides this search engine to allow searches of pharmacist licensure information by a combination of social security number along with license number or last name. The licensure information is computer generated and subject to change as licensure status changes. This website is a primary source verification, as required by JCAHO, of licensure status for all pharmacist licenses issued in the state of North Carolina.
